Notice to Patients with Dental Insurance Recently we have seen many changes with dental insurance plans regarding the frequency of polishing, fluoride, follow up exams, etc. Some plans are now only covering these services once every 9 months or yearly, when they were covered 6 months in the past. Also, you may only be covered for limited scaling each year. We stress that you are responsible for your own dental insurance, so it is important that you check with your insurance carrier to see if there have been any changes to your policy, prior to your appointment. Insurance companies do not inform dental offices of changes to individual dental plans. We the care givers, will continue with the treatment prescribed and previously discussed with you for your specific needs, and deliver the quality care that you require unless we are given instructions by you to cease your recommended treatment due to finances. As a courtesy we do direct assign some benefits, but you, the patient, are responsible for any amount that your insurance does not cover. If your insurance has not been paid within 60 days, the balance becomes your responsibility. Financial Arrangements for your Dental Treatment
