Notice to Patients with Dental Insurance

Recently we have seen many changes with dental insurance plans regarding the frequency of polishing, fluoride, follow up exams, etc. Some plans are now only covering these services once every 9 months or yearly, when they were covered 6 months in the past. Also, you may only be covered for limited scaling each year.

We stress that you are responsible for your own dental insurance, so it is important that you check with your insurance carrier to see if there have been any changes to your policy, prior to your appointment. Insurance companies do not inform dental offices of changes to individual dental plans. We the care givers, will continue with the treatment prescribed and previously discussed with you for your specific needs, and deliver the quality care that you require unless we are given instructions by you to cease your recommended treatment due to finances.

As a courtesy we do direct assign some benefits, but you, the patient, are responsible for any amount that your insurance does not cover. If your insurance has not been paid within 60 days, the balance becomes your responsibility.

Financial Arrangements for your Dental Treatment

  • All fees are due and payable the day services are rendered unless other arrangements have been made prior to any treatment.
  • For patients with insurance, once we have been able to verify your coverage with a written estimate, you can simply pay any applicable deductible and co-payment at each visit.
  • For major treatment (i.e. bridges, crowns, veneers, dentures), 50% of the patient total portion is due at the first appointment and the remainder patient balance is to be paid at the time of delivery.
  • We will do our very best to estimate for you, what your insurance coverage will be, but in all cases, the patient is responsible for the portion not paid by insurance. All insurance balances over 60 days are to be paid by the patient.
  • We accept cash, cheque, credit cards, or debit cards for your convenience. Please speak with your account manager anytime you have any questions about financial arrangements. All balances over 60 days are subject to a service charge of 2.0% per month
